A rare spinal granulocytic sarcoma (GS) can precede acute myelogenous leukemia. Early diagnosis of this epidural mass is crucial for timely treatment and preventing leukemic transformation.

Area of Science:

  • Oncology
  • Hematology
  • Neurology

Background:

  • Spinal epidural granulocytic sarcoma (GS) is a rare extramedullary manifestation of myeloid leukemia.
  • It can present as a mass causing spinal cord compression.

Observation:

  • A 10-year-old boy presented with leg weakness, initially misdiagnosed as histiocytic lymphoma.
  • Bone marrow involvement was absent at initial presentation.

Findings:

  • Retrospective immunoperoxidase staining for lysozyme confirmed spinal epidural GS fourteen months later.
  • The patient subsequently developed acute myelogenous leukemia.

Implications:

  • Spinal epidural GS should be considered in the differential diagnosis of epidural masses, even without overt leukemia.
