Insights

This study details a successful treatment for an elderly patient with hypertrophic cardiomyopathy. Myocardial contrast echocardiography-guided septal ablation offered an effective alternative when medical therapy failed.

Area of Science:

  • Cardiology
  • Medical Imaging
  • Interventional Cardiology

Background:

  • Hypertrophic cardiomyopathy (HCM) is a genetic heart muscle disease.
  • Left ventricular concentric hypertrophy with septal thickening is a common echocardiographic finding in HCM.
  • Medical management of symptomatic HCM can be challenging, with some patients showing suboptimal responses.

Observation:

  • An elderly patient with hypertension and significant left ventricular hypertrophy, specifically a sigmoid septum, exhibited poor tolerance to standard medical treatments.
  • The patient's condition necessitated exploring alternative therapeutic strategies.

Findings:

  • Percutaneous transluminal septal myocardial ablation (PTSMA) guided by myocardial contrast echocardiography (MCE) was successfully performed.
  • This minimally invasive procedure effectively addressed the patient's obstructive hypertrophic cardiomyopathy.

Implications:

  • MCE-guided PTSMA represents a viable and effective treatment option for select patients with obstructive HCM who are refractory to medical therapy.
  • This approach may offer improved clinical outcomes and quality of life in challenging HCM cases.
  • Further research into MCE-guided interventions for HCM is warranted.
