Summary

Background:

A 63-year-old female presented with a 7-year history of recurrent palpitations, sweating, and occasional syncope.
These episodes were associated with documented hypoglycemia (blood glucose 46 mg/dl), leading to clinical suspicion of insulinoma.

Observation:

Computed tomography revealed a retroperitoneal tumor.
The tumor's location was unusual for a typical insulinoma.

Findings:

Histological examination of the resected specimen confirmed the retroperitoneal tumor to be a paraganglioma.
This diagnosis contrasted with the initial clinical suspicion of insulinoma based on hypoglycemic symptoms.

Implications:

This case highlights the importance of considering rare tumor types in the differential diagnosis of hypoglycemia.
