Summary

Laparoscopic appendectomy (LA) in children offers shorter hospital stays and earlier oral intake compared to open appendectomy (OA). While complication rates were similar, LA is a safe alternative for suspected appendicitis, especially in certain patient groups.

Background:

  • Appendectomy is a common surgical procedure for appendicitis.
  • Laparoscopic appendectomy (LA) is increasingly used as an alternative to open appendectomy (OA).

Purpose of the Study:

  • To compare the outcomes of laparoscopic appendectomy (LA) versus open appendectomy (OA) in pediatric patients.
  • To evaluate operative time, hospital stay, complication rates, and postoperative recovery between LA and OA.

Main Methods:

  • A retrospective review of 113 pediatric patients (<18 years) undergoing appendectomy between July 2001 and July 2002.
  • Patients were divided into two groups: OA (N=59) and LA (N=54).
  • Comparison of operative time, hospital stay, complication rates, and time to oral intake.

Main Results:

  • Operative times were similar between OA and LA groups (P=0.328).
  • LA patients had significantly shorter hospital stays (3.07 days) than OA patients (4.69 days) (P=0.002).
  • Postoperative complication rates were not significantly different (OA 6.8% vs. LA 3.5%, P=0.681).
  • LA patients could drink earlier (12-24 hours) than postflatus OA patients (3.2 days).
  • OA group had a higher rate of ruptured appendicitis (13/59 vs. 2/54).

Conclusions:

  • Laparoscopic appendectomy is a safe procedure in children, associated with shorter hospital stays and faster return to oral intake compared to open appendectomy.
  • LA is particularly suitable for overweight adolescent females and cases of suspected appendicitis.
  • LA is not recommended for patients with an appendiceal mass.
