Background:

  • Mesothelial/monocytic incidental cardiac excrescence (MICE) is a rare intracardiac lesion.
  • It can lead to severe disease and misdiagnosis, potentially mimicking thrombus or neoplasia.

Observation:

  • A case involving a 66-year-old woman with pre-existing cardiac conditions is presented.
  • A lesion initially presumed to be a thrombus was discovered in the left atrium during surgery.

Findings:

  • Histopathological analysis revealed mesothelial cells (keratin/vimentin coexpression) and histiocytes (lysozyme/vimentin positive) embedded in fibrin.
  • This confirmed the diagnosis of cardiac MICE.

Implications:

  • Cardiac MICE is a benign lesion but can cause significant morbidity and mortality through embolization, leading to myocardial or multiorgan infarctions.
