Background:

African Americans experience disproportionately high rates of hypertension and type 2 diabetes.
Managing blood pressure and HbA1C is crucial for preventing cardiovascular complications in this population.

Purpose of the Study:

To assess the effectiveness of a community-based intervention program in reducing blood pressure and HbA1C levels.
To evaluate health improvements in a predominantly African American cohort.

Main Methods:

Seventy-five African American volunteers participated in a multi-component intervention.
Program elements included exercise, nutrition education, support groups, and service coordination.

Lifestyle modifications and social support are key components for managing chronic conditions in vulnerable populations.
