Neurogenic orthostatic hypotension (NOH) patients experienced improved blood pressure and standing tolerance with L-threo-3,4-dihydroxyphenylserine (L-DOPS) treatment. This synthetic amino acid converts to norepinephrine (NE), boosting blood pressure effectively.
Area of Science:
- Neuroscience
- Pharmacology
- Cardiovascular Medicine
Background:
- Neurogenic orthostatic hypotension (NOH) is characterized by insufficient norepinephrine (NE) availability in the synaptic cleft, impairing blood pressure regulation upon standing.
- This deficiency significantly impacts patients' ability to maintain blood pressure in an upright posture.
Purpose of the Study:
- To investigate the efficacy of L-threo-3,4-dihydroxyphenylserine (L-DOPS) in improving blood pressure and orthostatic tolerance in patients with severe NOH.
- To determine the mechanism by which L-DOPS exerts its pressor effects.
Main Methods:
- A dose-titration study identified optimal L-DOPS dosage in 19 NOH patients (8 with pure autonomic failure, 11 with multiple-system atrophy).
- A double-blind, placebo-controlled, crossover trial assessed the effects of L-DOPS on blood pressure and orthostatic tolerance.
- Plasma NE levels and the impact of carbidopa (an L-aromatic amino acid decarboxylase inhibitor) on L-DOPS response were evaluated.
Main Results:
- Oral L-DOPS administration significantly increased mean blood pressure in both supine and standing positions for several hours.
- Orthostatic tolerance was improved in all participating patients following L-DOPS treatment.
- Blood pressure increases correlated with elevated plasma NE levels, and carbidopa administration blunted both NE increase and pressor response.
Conclusions:
- Acute administration of L-DOPS effectively increases blood pressure and enhances orthostatic tolerance in NOH patients.
- The therapeutic effect of L-DOPS is attributed to its conversion into norepinephrine (NE) outside the central nervous system.
