Area of Science:

  • Endocrinology
  • Surgical Pathology
  • Medical Imaging

Background:

  • Ectopic thyroid tissue, particularly dual occurrences, represents a rare congenital anomaly.
  • Thyroid tissue located outside the normal anatomical position can present diagnostic challenges.

Observation:

  • A 20-year-old female presented with a dual ectopic thyroid, with tissue found in the lingual and infrahyoid areas.
  • Physical examination revealed anterior midline neck swelling and a base-of-tongue mass, with no thyroid tissue in the orthotopic location.
  • Thyroid function tests (Triiodothyronine, Thyroxine, Thyroid-Stimulating Hormone) were within normal limits.

Findings:

  • A technetium-99m sodium pertechnetate thyroid scan confirmed the presence of dual ectopic thyroid tissue.
  • The scan demonstrated a lack of iodine uptake in the normal thyroid gland location.

Related Experiment Videos

  • Surgical removal of the infrahyoid ectopic thyroid was performed for cosmetic reasons; the symptomatic lingual thyroid was managed conservatively.
  • Implications:

    • Thyroid scintigraphy is crucial for evaluating anterior midline neck swellings, especially when ectopic thyroid is suspected.
    • Management strategies for ectopic thyroid tissue should consider patient symptoms, cosmetic concerns, and anatomical location.
    • This case underscores the variability in presentation and management of rare ectopic thyroid conditions.
