Background:

Echinococcosis is a parasitic disease caused by tapeworms of the genus Echinococcus.
Pelvic echinococcal cysts are rare, often presenting as pelvic masses.
Transvaginal ultrasound is a primary imaging modality for pelvic masses.

Observation:

A patient presented with a pelvic mass initially suspected to be a multicystic ovary on transvaginal ultrasound.
A concurrent liver mass raised suspicion for echinococcosis.
Open surgical removal of the pelvic cyst was performed.

Findings:

Pathological examination confirmed the pelvic mass as an echinococcus cyst.
The case underscores the importance of considering echinococcosis in the differential diagnosis of pelvic masses.
