Summary

This case report highlights an unusual presentation of Crohn's disease in a child, where recurrent lip swelling (cheilitis) preceded significant intestinal symptoms. Physicians should consider Crohn's disease in pediatric patients with persistent cheilitis.

Area of Science:

  • Pediatric Gastroenterology
  • Immunology
  • Dermatology

Background:

  • Crohn's disease is a chronic inflammatory bowel disease.
  • Its presentation can be variable, especially in pediatric cases.
  • Extraintestinal manifestations are common but can precede gastrointestinal symptoms.

Observation:

  • A five-year-old boy presented with painless bilateral lip swelling.
  • He later developed abdominal pain, diarrhea, weight loss, and joint pain.
  • Colonoscopy revealed findings consistent with Crohn's colitis.

Findings:

  • The patient experienced relapsing cheilitis, particularly when steroid treatment for Crohn's disease was tapered.
  • Intestinal symptoms of Crohn's disease were minimal during these relapses.

Related Experiment Videos

  • Colonic biopsies confirmed chronic inflammation, cryptitis, and fissure formation.
  • Implications:

    • This case underscores the importance of recognizing cheilitis as a potential early or isolated manifestation of pediatric Crohn's disease.
    • Physicians should maintain a high index of suspicion for inflammatory bowel disease in children with recurrent or persistent cheilitis.
    • Prompt diagnosis and management of Crohn's disease, even with atypical presentations, are crucial for long-term patient outcomes.
