Background:

  • Postpartum thyroiditis (PPT) is an autoimmune thyroid disorder occurring after childbirth.

Observation:

  • A 30-year-old woman presented with a solitary anterior neck nodule five months postpartum.
  • Initial diagnosis was PPT based on laboratory tests, ultrasound, and fine-needle aspiration biopsy.
  • Twenty-eight months postpartum, she developed overt hyperthyroidism, diagnosed as Graves' disease.

Findings:

  • The PPT initially presented as a 'cold' nodule on technetium-99m thyroid scan.
  • Elevated thyroid peroxidase and TSH receptor antibodies confirmed Graves' disease.
