Summary

Blastocyst (day 5) versus day 3 embryo transfer showed similar implantation and pregnancy rates in a randomized study. This suggests day 5 transfer offers no significant advantage when equal numbers of viable embryos are transferred.

Background:

  • Limited prospective randomized studies exist comparing day 3 and day 5 embryo transfers.
  • Previous studies often transferred more embryos on day 3 than blastocysts on day 5, confounding comparisons.

Purpose of the Study:

  • To compare the implantation and pregnancy potential of embryos transferred on day 3 versus day 5.
  • To evaluate if blastocyst transfer offers an advantage over day 3 transfer under specific conditions.

Main Methods:

  • A prospective randomized study involving 118 patients undergoing standard IVF/intracytoplasmic sperm injection.
  • Patients with at least three 8-cell embryos (<20% fragmentation) on day 3 were randomized for day 3 or day 5 transfer.

    • Embryo transfer on day 3 or day 5 yields similar implantation rates when an equal number of viable embryos are transferred.
    • Blastocyst stage transfer does not appear to offer a significant advantage over day 3 transfer for patients with multiple good-quality embryos available on day 3.
