Chronic high fatigue may indicate underlying sleep pathology. This study found significantly more sleep disorders in highly fatigued miners, suggesting fatigue and sleepiness are independent and warrant separate assessment.
Area of Science:
- Occupational Health
- Sleep Medicine
- Clinical Research
Background:
- Chronic fatigue affects 20-25% of primary healthcare patients, often overlooked compared to sleepiness.
- Shift workers face heightened risks of fatigue-related morbidity and mortality.
- Identifying the root causes of chronic fatigue is crucial for effective clinical management.
Purpose of the Study:
- To investigate if fatigue severity can independently predict underlying sleep pathology.
- To differentiate between subjective fatigue and objective sleepiness.
- To assess the prevalence of sleep disorders in relation to fatigue levels.
Main Methods:
- A cross-sectional study involving 195 underground miners in Timmins, Ontario.
- Selection of 21 most-fatigued and 23 least-fatigued subjects based on the Fatigue Severity Scale (FSS).
- Polysomnographic evaluation to diagnose sleep disorders in selected groups, matched for age, gender, and BMI.
Main Results:
- 71.4% of the most-fatigued group exhibited significant sleep pathology, versus 13.0% in the least-fatigued group (P < 0.0001).
- A weak correlation (Pearson's r = 0.45) was observed between subjective fatigue and sleepiness in the total cohort.
- Fatigue Severity Scale scores were significantly higher in the fatigued group (4.9 ± 0.5) compared to the control group (2.2 ± 0.5) (P < 0.0001).
Conclusions:
- Chronic high fatigue can serve as an independent indicator of underlying sleep pathology.
- Subjective fatigue and sleepiness appear to be distinct phenomena requiring separate assessment.
- Independent subjective and objective evaluations are warranted for individuals presenting with chronic fatigue.
