Background:

Actinic superficial folliculitis (ASF) is a rare condition, with only three prior reports since its initial description in 1985.
The clinical and histopathological characteristics of ASF suggest it may be a distinct dermatological entity.

Observation:

A 30-year-old male developed superficial follicular pustules on his back, shoulders, and chest following intense heat and sun exposure.
Pustule formation occurred within 24-36 hours of exposure.

Findings:

Histology confirmed subcorneal pustules and suppurative folliculitis.
Immunohistochemistry revealed a perivascular and perifollicular inflammatory infiltrate composed of T cells, macrophages, and neutrophils.

Implications:

This case represents the fourth reported instance of ASF and the first to characterize its inflammatory infiltrate.
