Summary

Relapsing polychondritis (RPC) in Orientals shows similar symptoms to Caucasians but with fewer skin, kidney, or nervous system issues. Airway complications are more frequent and severe in Oriental patients with RPC.

Background:

  • Relapsing polychondritis (RPC) is a rare multisystem inflammatory disease.
  • Most documented cases of RPC are from Caucasian populations, with limited data on other ethnic groups.

Purpose of the Study:

  • To investigate the clinical features, management strategies, and outcomes of relapsing polychondritis (RPC) in an Oriental population.
  • To compare the presentation and course of RPC in Oriental patients with existing literature from Caucasian cohorts.

Main Methods:

  • A retrospective review of medical records for patients diagnosed with relapsing polychondritis (RPC) between 1989 and 2001.
  • Inclusion criteria required fulfillment of the McAdam-Michet-Damiani-Levine diagnostic criteria for RPC.

  • The study identified 12 Oriental patients (10 Chinese, 2 Malay) with RPC, a female-to-male ratio of 3:1, and a mean age of onset of 34 years.
  • Common manifestations included auricular (10), articular (9), and ocular (8) involvement. Notably, cutaneous, renal, and central nervous system involvement were absent.
  • Serious airway complications, such as laryngotracheal involvement (6), were frequent, necessitating tracheostomy (5) and airway stenting (1).

Conclusions:

  • Relapsing polychondritis (RPC) in Oriental populations shares clinical similarities with Caucasian cohorts.
  • However, Oriental patients may present with a lower incidence of cutaneous, renal, or nervous system involvement.
  • A higher prevalence of severe airway complications, including laryngotracheal involvement, is a significant characteristic in Oriental RPC patients.
