Area of Science:

  • Hepatology
  • Toxicology
  • Dermatology

Background:

  • Flutamide is an antiandrogen medication prescribed for conditions like hirsutism (hair loss).
  • Drug-induced liver injury (DILI) is a significant concern with various medications.
  • Hirsutism affects women and can be treated with antiandrogens.

Observation:

  • A 41-year-old woman developed deep jaundice after three months of flutamide therapy for hair loss.
  • The patient experienced hepatic encephalopathy, a serious complication of liver dysfunction.
  • Symptoms improved significantly after flutamide was discontinued.

Findings:

  • Flutamide administration was temporally associated with the onset of severe hepatotoxicity.
  • The clinical presentation included cholestatic jaundice and signs of acute liver failure.
  • Discontinuation of flutamide led to the resolution of liver injury and encephalopathy.

Implications:

  • Flutamide carries a risk of severe, potentially fatal, toxic liver injury.
  • Routine monitoring of liver function tests is essential for patients on flutamide.
  • Early recognition and cessation of the offending drug are critical for managing flutamide-induced liver injury.
