This case study highlights a fatal outcome of severe ascending cholangitis. Aggressive endoscopic and surgical interventions for biliary necrosis and perforation were unsuccessful, potentially worsening the patient's condition.

Background:

  • Ascending cholangitis is a serious bacterial infection of the biliary system.
  • Necrosis and perforation of the biliary system represent a critical, life-threatening complication.

Observation:

  • A 40-year-old male presented with symptoms of acute cholangitis, including abdominal pain, fever, and jaundice.
  • Extensive suppurative pancholangitis complicated surgical intervention, preventing adequate biliary drainage.

Findings:

  • Despite endoscopic retrograde cholangiogram and surgical attempts, the patient succumbed to the infection two days post-operatively.
  • The severity of the pancholangitis precluded successful management, indicating a poor prognosis in such advanced cases.
