Infection after shoulder instability surgery is rare but can occur early or be delayed. Late infections may involve retained sutures, but recurrent instability was not observed in this patient series.

Area of Science:

  • Orthopedic Surgery
  • Infectious Diseases

Background:

  • No prior series documented outcomes of post-surgical shoulder instability infections.
  • This study aimed to evaluate functional outcomes and causative organisms in infected patients.

Observation:

  • Six patients treated for infection after shoulder instability surgery (1980-2001).
  • Three early (≤6 weeks) and three late (>8 months) infections observed.
  • Late infections were associated with sinus tracts and retained nonabsorbable sutures.

Findings:

  • No recurrent shoulder instability occurred in any patient.
  • Three polymicrobial and three single-organism infections were identified.
  • Infections can manifest acutely or be significantly delayed.

Implications:

  • High suspicion for retained nonabsorbable sutures is warranted in late post-surgical infections.
  • Understanding infection timing and causes aids in patient management.
  • Further research on preventing and treating these infections is needed.
