Summary

A rare case of primary epididymal leiomyoma, a benign tumor originating from the epididymis, was diagnosed in a 53-year-old man. This painful scrotal mass required inguinal orchiectomy for removal.

Background:

Benign tumors of the epididymis are rare, with leiomyomas being exceptionally uncommon.
Accurate preoperative diagnosis of paratesticular masses can be challenging due to their location and potential adhesion to testicular structures.

Observation:

A 53-year-old male presented with a painful, indurated left scrotum, making it difficult to distinguish the origin of the mass via palpation.
Imaging studies (CT and ultrasound) identified a 3 cm paratesticular mass adjacent to an atrophic left testis, suspected to arise from the epididymis.

Findings:

Surgical exploration and subsequent histopathological examination confirmed the mass as a primary leiomyoma of the epididymis, specifically originating from the epididymal tail.
The tumor measured 5 x 3 x 2 cm and was characterized as opalescent, gum elastic, and solid.
