Summary

Gene therapy using interleukin (IL)-1 receptor antagonist (IL-1RA) shows promise for rheumatoid arthritis (RA) treatment. Intraarticular gene transfer in patients demonstrated safety and therapeutic protein expression, paving the way for future efficacy studies.

Background:

  • Conventional rheumatoid arthritis (RA) treatments face challenges in drug delivery.
  • Intraarticular gene transfer offers a novel approach for localized therapeutic protein expression.

Purpose of the Study:

  • To assess the safety and feasibility of gene therapy for rheumatoid arthritis (RA).
  • To evaluate intraarticular expression of interleukin (IL)-1 receptor antagonist (IL-1RA) via gene transfer in RA patients.

Main Methods:

  • Autologous synoviocytes were retrovirally transduced to express IL-1RA.
  • Transduced synoviocytes were transplanted into metacarpophalangeal (MCP) joints in a clinical trial.
  • Articular tissues and fluids were analyzed for IL-1RA transgene expression using RT-PCR and in situ hybridization.
