Summary

Early rectal carcinoma with deep submucosal invasion, particularly subtypes IIa+IIc, shows lymphovascular infiltration. Major surgery is recommended due to potential lymph node metastasis risk, even without increased node count.

Area of Science:

  • Oncology
  • Surgical Pathology
  • Gastroenterology

Context:

  • Review of early rectal carcinoma cases (1981-1991).
  • Focus on histopathological examination of lymph nodes.
  • Analysis of tumor characteristics and surgical outcomes.

Purpose:

  • To determine lymph node involvement in early rectal carcinoma.
  • To assess the implications of lymph node status for treatment.
  • To correlate tumor features with lymphovascular infiltration and metastasis risk.

Summary:

  • Sixteen early rectal carcinoma patients analyzed for lymph node involvement.
  • Tumor predominantly in the lower rectum; polypoid and flat-elevated ulcerated subtypes noted.
  • Close relation found between submucosal invasion depth, tumor subtype (IIa+IIc), and lymphovascular infiltration.

Impact:

  • Suggests major surgical resection for deep submucosal invasion (Sm1c or deeper).
  • Highlights risk of lymph node metastasis even without increased lymph node count.
  • Informs treatment decisions for early rectal cancer based on histopathological findings.
