Background:

  • Traditional epidural recording for spinal cord function is invasive.
  • Developing non-invasive methods for spinal cord evaluation is crucial for patient comfort and safety.

Observation:

  • Surface spinal evoked potentials (SSEPs) were recorded from the cervical skin surface (C3, C5, C7) following median nerve stimulation.
  • SSEPs were compared to traditional epidural recordings in 18 healthy individuals and 37 patients with cervical lesions.
  • Normal SSEPs exhibit three negative waves (N1, N2, N3).

Findings:

  • Abnormal N2 (80%) and N3 (100%) waves were detected in cervical myelopathy patients.
  • Abnormal N2 waves were exclusively observed in radiculopathy patients, enabling differentiation.
  • Improvements in SSEPs correlated directly with clinical symptom improvement post-operatively.
