Background:

Cytomegalovirus (CMV) infections typically manifest with broader gastrointestinal involvement.
Isolated duodenal CMV disease is an uncommon clinical presentation.

Two patients presented with distinct duodenal pathologies attributed to CMV.
One patient experienced ulcer-type dyspepsia, while the other suffered severe gastrointestinal bleeding.

Findings:

Endoscopic evaluation with mucosal biopsy is crucial for diagnosing isolated duodenal CMV.
Ganciclovir therapy demonstrated efficacy, leading to clinical and endoscopic remission in both cases.

Implications:

This case series underscores the importance of considering CMV in unexplained duodenal diseases.
