    Background:

    • Congestive heart failure (CHF) can have various causes, including coronary heart disease and valve disease.
    • Idiopathic dilated cardiomyopathy presents a diagnostic challenge when no clear etiology is identified.

    Observation:

    • A 45-year-old woman presented with CHF without identifiable cardiac disease.
    • She had a history of prolonged high-dose dextroamphetamine use.

    Findings:

    • Dextroamphetamine-induced cardiomyopathy was diagnosed.
    • Cardiac dependence on dextroamphetamine was suggested by deterioration upon withdrawal.
    • Similarities to myocarditis seen in pheochromocytoma were noted.

    Implications:

    • Dextroamphetamine is a potential cause of cardiomyopathy.
    • Physicians should consider drug-induced cardiomyopathy in patients with unexplained heart failure.
    • Further research into amphetamine's cardiotoxic mechanisms is warranted.
