Summary

Cysteinyl-leukotrienes (LTC4, LTD4, LTE4) are not significantly elevated in exercise-induced asthma, unlike in allergen-induced asthma. This suggests these mediators play a minor role in exercise-induced bronchoconstriction.

Background:

  • Cysteinyl-leukotrienes (LTC4, LTD4, LTE4) are implicated in asthma pathogenesis.
  • Exercise-induced bronchoconstriction (EIB) is a common asthma complication.
  • The role of cysteinyl-leukotrienes in EIB requires further elucidation.

Purpose of the Study:

  • To investigate the role of cysteinyl-leukotrienes in exercise-induced asthma.
  • To compare urinary LTE4 excretion after exercise challenge versus methacholine challenge in asthmatic subjects.

Main Methods:

  • Nine asthmatic subjects with EIB underwent treadmill exercise and methacholine challenge on separate occasions.
  • Maximal bronchoconstriction (FEV1 fall) was matched between challenges.

  • Urinary LTE4 excretion was measured using HPLC-RIA in two 90-min fractions post-challenge.
  • Main Results:

    • No significant differences in urinary LTE4 excretion were observed between exercise and methacholine challenges.
    • LTE4 levels remained comparable in both 0-90 min and 90-180 min post-challenge periods.
    • The overall 0-180 min urinary LTE4 excretion showed no significant variation between the two challenge types.

    Conclusions:

    • Enhanced cysteinyl-leukotriene generation, as assessed by urinary LTE4, is unlikely to be a major factor in exercise-induced bronchoconstriction.
    • Unlike allergen-induced bronchoconstriction, EIB does not appear to involve significant systemic release of cysteinyl-leukotrienes.
    • Any local cysteinyl-leukotriene activity in EIB is likely of insufficient magnitude to alter urinary LTE4 levels.
