Summary

High-dose chemotherapy, particularly with carboplatin, offers long-term survival for patients with recurrent or refractory germ cell cancer. This intensive approach is now a key salvage therapy and considered for initial treatment in high-risk cases.

Area of Science:

  • Oncology
  • Pharmacology

Background:

  • Dose-intensive chemotherapy for germ cell cancer has advanced significantly.
  • Early high-dose regimens showed response but lacked durability.

Purpose of the Study:

  • To evaluate the efficacy of second-generation, germ cell cancer-specific high-dose chemotherapy regimens.
  • To assess the impact of carboplatin and cisplatin in advanced germ cell cancer.

Main Methods:

  • Review of large studies from Indiana University and European centers.
  • Analysis of high-dose chemotherapy regimens incorporating carboplatin or cisplatin.

Main Results:

  • High-dose carboplatin or cisplatin-based regimens achieve long-term survival in previously incurable patients.
  • High-dose carboplatin is emerging as a crucial first salvage therapy.
  • This approach is being used in initial therapy for poor-risk patients.

Conclusions:

  • High-dose carboplatin-based chemotherapy is effective for recurrent/refractory germ cell cancer.
  • The success of these protocols is leading to investigations in other cancers like ovarian cancer, neuroblastoma, lymphoma, and breast cancer.
