Summary

Small airways obstruction can occur before chronic lung disease is apparent. Time domain indices are sensitive indicators of this obstruction, especially in silica-exposed workers with increasing dust exposure.

Area of Science:

  • Occupational Medicine
  • Pulmonary Physiology
  • Environmental Health

Background:

  • Small airways obstruction can precede overt chronic airway obstruction.
  • Spirometric indices, particularly flow rates at low lung volumes, can indicate small airways status.
  • Time domain indices, derived from spirogram moments, are sensitive to small airways obstruction.

Purpose of the Study:

  • To evaluate small airways function in granite quarry workers exposed to dust.
  • To assess small airways function in relation to dust exposure levels.
  • To determine if small airways obstruction is present in workers without silicosis or large airway obstruction.

Main Methods:

  • Applied spirometric and time domain indices to 140 granite quarry workers.

Related Experiment Videos

  • Digitized volume-time spirograms using an electronic digitizer and microcomputer.
  • Computed flow and time domain indices, categorizing workers into three dust exposure groups.
  • Main Results:

    • All time domain indices showed significant small airways obstruction correlating with increased dust exposure, after adjusting for age, height, and smoking status.
    • Smokers exhibited greater airway obstruction than non-smokers.
    • A trend of increasing small airways obstruction was observed with higher dust exposure levels.

    Conclusions:

    • Small airways obstruction is prevalent in silica-exposed workers, even without radiological silicosis or large airway obstruction.
    • Dust exposure is directly related to the degree of small airways obstruction.
    • Time domain indices demonstrate higher sensitivity for detecting small airways obstruction compared to other spirometric measures.
