Summary

Internalized capillaries in muscle fibers are linked to exertional myalgia in males. This finding suggests a potential ischemic cause for capillary changes, mitochondrial inclusions, and muscle pain.

Background:

  • Exertional myalgia can stem from various underlying pathologies.
  • Capillary abnormalities in muscle tissue are indicative of potential circulatory issues.
  • Hereditary motor and sensory neuropathy (HMSN) Type 2 presents with specific neuropathic changes.

Observation:

  • Two adult males with exertional myalgia showed internalized capillaries in muscle fibers.
  • One patient had bundles of internalized capillaries in Type 1 fibers with cytoarchitectural changes.
  • The other patient, with HMSN Type 2, exhibited internalized capillaries alongside neuropathic changes.

Findings:

  • Electron microscopy revealed paracrystalline inclusions in the mitochondria of both patients' muscle.
  • Dystrophin levels were normal, ruling out muscular dystrophy as the cause.
  • A significant majority (65/77) of patients with capillary internalization are male, with 10 reporting myalgia or cramps.

Implications:

  • Capillary internalization may be associated with exertional myalgia and mitochondrial abnormalities.
  • An ischemic predisposition is proposed as a potential trigger for these observed changes.
  • Further research into the link between ischemia, capillary internalization, and myalgia is warranted.
