    Summary

    Immunological changes, including autoantibodies and immune complexes, preceded proteinuria in gold-induced nephropathy. These immune markers appeared before kidney complications, suggesting a role in disease development.

    Background:

    • Gold therapy can induce nephropathy, a kidney disease.
    • The immunological mechanisms underlying gold-induced nephropathy are not fully understood.

    Observation:

    • Immunological tests on a patient with gold-induced nephropathy revealed pre-proteinuria phenomena.
    • These included the formation of tissue antibodies, rheumatoid factors, and circulating immune complexes.

    Findings:

    • An antigen sharing determinants with DOC-extractable tissue antigens was detected in circulation before and during proteinuria.
    • Precipitating antibodies against this antigen were found in serum samples obtained weeks before diagnosis.
    • Antibodies reacting with various tissue extracts (saline, DOC, BE preparations) were also identified, diminishing before proteinuria onset.

    Implications:

    • These immunological events may play a significant role in the pathogenesis of gold-induced nephropathy.
    • Early detection of these immune markers could potentially aid in predicting or diagnosing kidney complications associated with gold treatment.
