Summary

Lactic acidosis significantly increases in patients with chronic obstructive pulmonary disease (COPD) during incremental exercise tests. This rise in blood lactate correlates with changes in ventilation and carbon dioxide output.

Area of Science:

  • Respiratory Medicine
  • Exercise Physiology
  • Biochemistry

Context:

  • Chronic obstructive pulmonary disease (COPD) is characterized by airflow limitation and systemic effects.
  • Incremental exercise testing is used to assess functional capacity and physiological responses in various populations.
  • Lactic acidosis, a metabolic consequence of anaerobic metabolism, is not well-characterized during exercise in COPD patients.

Purpose:

  • To investigate the development of lactic acidosis during incremental exercise in individuals with COPD.
  • To determine the relationship between exercise-induced lactate changes and cardiorespiratory parameters in COPD patients.

Summary:

  • Twelve untrained subjects with COPD underwent maximal incremental cycle ergometer tests.
  • Measurements included ventilation (VE), oxygen uptake (VO2), carbon dioxide output (VCO2), end-tidal CO2 (FETCO2), arterial lactate, and blood gases.

Related Experiment Videos

  • Significant increases in blood lactate were observed during maximal exercise, correlating with changes in VE, VCO2, and respiratory quotient (RQ).
  • T40 and base excess extracellular fluid (BEecf) also showed strong correlations with lactate changes.
  • Impact:

    • This study demonstrates that lactic acidosis is a notable physiological response to incremental exercise in COPD.
    • Findings suggest that changes in VE, VCO2, and RQ can serve as indicators of increased lactate levels during exercise in COPD.
    • Understanding these metabolic responses can inform exercise prescription and management strategies for COPD patients.
