Summary

Very low calorie diets (VLCD) achieve rapid weight loss, but maintaining it is difficult. Dexfenfluramine (D-F) combined with VLCD significantly enhances long-term weight management compared to placebo.

Area of Science:

  • Obesity research
  • Pharmacology
  • Dietary interventions

Background:

  • Very low calorie diets (VLCD) facilitate significant initial weight loss but long-term maintenance is a challenge.
  • Conventional behavioral modification techniques for weight management have limited applicability and success rates.
  • Pharmacological agents, like serotonin agonists, offer potential for sustained weight loss following VLCD.

Purpose of the Study:

  • To evaluate the efficacy of dexfenfluramine (D-F) in maintaining weight loss initiated by a VLCD.
  • To compare the long-term weight management effects of D-F versus placebo in patients who completed VLCD treatment.

Main Methods:

  • A double-blind, randomized controlled trial involving 45 patients who completed an 8-week VLCD.
  • Participants were assigned to receive either D-F (15 mg twice daily) or placebo for 26 weeks, while maintaining a reduced-calorie diet.

Related Experiment Videos

  • Weight changes were monitored throughout the VLCD phase and the subsequent drug treatment period.
  • Main Results:

    • Patients treated with D-F achieved an additional average weight loss of 5.8 kg, resulting in a total loss of 21.3 kg over 34 weeks.
    • The placebo group regained an average of 2.9 kg, with a total weight loss of 11.3 kg.
    • Switching to D-F in an open-label extension stabilized weight in patients who had regained weight on placebo.

    Conclusions:

    • Dexfenfluramine is effective in maintaining and enhancing weight loss achieved through very low calorie diets.
    • Pharmacological intervention with D-F provides a significant clinical benefit for long-term obesity management.
    • D-F offers a viable strategy for preventing weight regain after VLCD treatment.
