Summary

This study presents a novel variation of the total cavopulmonary connection, rerouting blood flow through an intra-atrial tunnel to the right pulmonary artery. This technique offers an alternative surgical approach for specific congenital heart conditions.

Area of Science:

  • Cardiovascular Surgery
  • Pediatric Cardiology
  • Congenital Heart Disease

Background:

  • The total cavopulmonary connection (TCPC) is a Fontan procedure variant for single-ventricle physiology.
  • Standard TCPC involves direct anastomosis of the superior vena cava to the right pulmonary artery.

Observation:

  • A modified TCPC technique is described, avoiding direct superior vena cava to right pulmonary artery anastomosis.
  • Blood from both vena cavae is directed via an intra-atrial tunnel.

Findings:

  • The intra-atrial tunnel directs flow to the right atrial appendage.
  • The right atrial appendage is then anastomosed to the right pulmonary artery.
  • This approach is feasible in anatomically suitable cases.

Implications:

  • This surgical variation may offer an alternative for complex Fontan-associated issues.
  • Further research is needed to evaluate long-term outcomes and patient selection criteria.
  • This technique expands the surgical options for single-ventricle palliation.
