Area of Science:

  • Cardiovascular Medicine
  • Rheumatology
  • Surgical Cardiology

Background:

  • Systemic lupus erythematosus (SLE) is an autoimmune disease associated with cardiovascular complications.
  • Long-term corticosteroid therapy, often used in SLE management, can increase risks for cardiovascular issues.
  • Hypertension is a common comorbidity in SLE patients.

Observation:

  • A 33-year-old female with a history of hypertension and SLE on high-dose corticosteroids presented with recurrent respiratory infections and congestive heart failure.
  • Diagnostic imaging revealed aortic insufficiency with aneurysmal dilation of Valsalva's posterior sinus.
  • Intraoperative findings confirmed aortic dissection causing aortic valve insufficiency.

Findings:

  • Surgical intervention successfully corrected the aortic dissection using a bovine pericardial tube and aortic valve suspension.

Related Experiment Videos

  • The patient experienced postoperative complications including seizures and hemiparesis, alongside a respiratory infection.
  • She was discharged with residual mild hemiparesis and in New York Heart Association functional class I.
  • Implications:

    • Aortic dissection should be considered in SLE patients presenting with aortic insufficiency, particularly those with hypertension and long-term corticosteroid use.
    • This case highlights the complex interplay between autoimmune disease, its treatment, and severe cardiovascular events.
    • Prompt diagnosis and surgical management are vital for improving outcomes in patients with this rare presentation.
