Summary

Histologic resolution of collagenous colitis is achievable with treatment, often leading to symptom relief. However, diagnostic biopsies may be patchy and inconsistently sampled, particularly in the rectosigmoid region.

Area of Science:

  • Gastroenterology
  • Pathology

Background:

  • Collagenous colitis is a condition characterized by a thickened subepithelial collagen band in the colon.
  • Understanding its histologic variability and correlation with clinical outcomes is crucial for effective management.

Purpose of the Study:

  • To evaluate the histologic features of collagenous colitis.
  • To correlate these histologic changes with disease behavior and treatment response.

Main Methods:

  • Analysis of 212 colon tissue specimens from 14 patients with collagenous colitis.
  • Independent, blinded interpretation by two pathologists.
  • Sequential evaluations over a mean follow-up of 33 months.

Main Results:

  • Histologic resolution occurred in 57% of patients after a mean of 14 months of therapy, with persistent symptoms in only 12%.

Related Experiment Videos

  • Histologic findings varied within the same anatomic region over time and between different regions during single examinations.
  • Normal mucosa was frequently found in rectosigmoid specimens, potentially leading to missed diagnoses in 40% of cases if relying solely on proctosigmoidoscopy.
  • Conclusions:

    • Histologic resolution of collagenous colitis is associated with symptom resolution.
    • Histologic features can be patchy and inconsistently sampled.
    • Rectosigmoid biopsies may underestimate the presence and extent of collagenous colitis.
