Partial weight bearing does not reliably unload healing bone in tibial osteotomies. Increased ground reaction forces were observed during weight-bearing activities, with no significant reduction in interfragmentary movements.

Area of Science:

  • Orthopedic surgery
  • Biomechanics
  • Bone healing

Background:

  • Partial weight bearing is a common strategy to protect healing bone.
  • Objective methods to assess weight-bearing load on healing bone are lacking.
  • Tibial osteotomies present challenges in managing healing and loading.

Purpose of the Study:

  • To investigate the effects of partial weight bearing on bone loading in tibial osteotomies.
  • To objectively measure interfragmentary movements and ground reaction forces during various weight-bearing conditions.

Main Methods:

  • Six patients with tibial osteotomies stabilized by Ilizarov fixators participated.
  • Three-dimensional interfragmentary movements were measured using reflective markers.
  • Ground reaction forces were assessed during rest, co-contraction, standing, slow walking, and 20 kg partial weight bearing.

Related Experiment Videos

Main Results:

  • Ground reaction forces significantly increased during partial weight bearing, standing, and walking compared to co-contraction (P<0.05).
  • Interfragmentary movements were comparable across partial weight bearing, standing, and walking conditions (P>>0.05).
  • No direct relationship was found between interfragmentary movement and ground reaction forces.

Conclusions:

  • Partial weight bearing does not reliably reduce loading in the defect zone of tibial osteotomies.
  • The concept of partial weight bearing may not effectively unload healing bone.
  • Weight-bearing activities can increase awareness and potentially prevent critical situations like stumbling.
