Area of Science:

  • Oncology
  • Pathology

Background:

  • Prostate carcinosarcoma is an exceptionally rare malignancy.
  • This case highlights a unique presentation and metastatic behavior of this rare tumor.

Observation:

  • A 77-year-old male presented with primary and metastatic carcinosarcoma of the prostate.
  • The patient exhibited resistance to anti-androgen therapy.
  • Metastases were widespread, including lung, liver, pancreas, lymph nodes, spleen, and penis, but notably absent in bone.

Findings:

  • The carcinosarcoma demonstrated both sarcomatous (chondrosarcoma, fibrosarcoma) and carcinomatous components.
  • Immunohistochemistry revealed vimentin positivity in the sarcomatous elements and keratin and prostatic acid phosphatase positivity in the carcinomatous elements.

Implications:

  • This case expands the understanding of prostate carcinosarcoma's clinical behavior and metastatic potential.
  • The findings underscore the aggressive nature and treatment resistance of this rare prostate cancer subtype.
