  • Extragonadal germ cell tumors are uncommon, presenting diagnostic challenges.

Observation:

  • A 25-year-old unmarried female presented with a unilateral vulval swelling.
  • Initial examination revealed normal internal reproductive organs, including ovaries.

Findings:

  • The vulval swelling was diagnosed as an endodermal sinus tumor (EST) of extragonadal origin.
  • This represents an unusual presentation of EST outside the typical ovarian location.

Implications:

  • This case expands the known spectrum of extragonadal germ cell tumor locations.
  • Highlights the importance of considering rare diagnoses in gynecologic presentations.
  • Suggests the need for comprehensive evaluation in cases of unexplained vulval masses.
