Summary

Intractable hiccups can signal serious neurologic disorders, as seen in a 19-year-old man with Arnold-Chiari malformation and syringomyelia. Early diagnosis and treatment are crucial for managing these rare but severe conditions.

Area of Science:

  • Neurology
  • Neurosurgery

Background:

  • Hiccups (singultus) are typically transient and benign.
  • Rarely, persistent hiccups may indicate an underlying serious pathology.

Observation:

  • A 19-year-old male presented with intractable hiccups as the primary symptom.
  • Physical examination revealed right-sided vocal cord paralysis.

Findings:

  • Magnetic resonance imaging (MRI) demonstrated a Type I Arnold-Chiari malformation.
  • A large cervicothoracic syringomyelia was identified.
  • Ventriculoperitoneal shunt placement provided initial symptom relief.

Implications:

  • This case highlights the importance of investigating persistent hiccups for underlying neurological conditions.

Related Experiment Videos

  • Arnold-Chiari malformation and syringomyelia are critical differential diagnoses for intractable hiccups.
  • Prompt neurosurgical evaluation is essential for patients with concerning neurological signs accompanying hiccups.
