Summary

Severe underlying diseases and intensive therapies predispose patients to active tuberculosis (TB) in hospitals. This study highlights TB as a critical infection in compromised hosts, posing a significant clinical challenge.

Area of Science:

  • Infectious Diseases
  • Pulmonology
  • Oncology

Background:

  • Hospitalized patients with severe underlying conditions are susceptible to opportunistic infections.
  • Active tuberculosis (TB) poses a significant risk in immunocompromised individuals within healthcare settings.

Purpose of the Study:

  • To investigate the incidence, clinical features, and outcomes of active tuberculosis (TB) in hospitalized patients.
  • To identify predisposing factors and assess the impact of TB on patient mortality.

Main Methods:

  • Retrospective analysis of 21 hospitalized patients diagnosed with active tuberculosis (TB).
  • Diagnosis confirmed by bacteria-positive biopsy or autopsy.
  • Clinical data, including chest X-ray findings, underlying diseases, and treatments, were reviewed.

Related Experiment Videos

Main Results:

  • 21 patients developed active TB, with infections in the lungs, pleura, lymph nodes, and miliary patterns.
  • Unusual chest X-ray findings (lower lung field pneumonia, miliary pattern) were observed in half the patients.
  • Severe underlying diseases and treatments (steroids, immunosuppressants, chemotherapy, radiation, surgery) were identified as predisposing factors.
  • 14 patients died, with TB directly causing death in 8 cases.

Conclusions:

  • Active tuberculosis is a critical opportunistic infection in hospitalized patients with compromised immune systems.
  • Intensive medical therapies and severe underlying conditions significantly increase TB risk.
  • Early recognition and management of TB are crucial in general hospitals to improve outcomes for vulnerable patients.
