Summary

Left ventricular involvement is common in right ventricular cardiomyopathy, often progressing over time. This study highlights the need to monitor both ventricles in patients diagnosed with this condition.

Background:

Right ventricular cardiomyopathy (RVC) is typically viewed as affecting only the right ventricle.
However, some reports suggest concurrent left ventricular (LV) abnormalities exist.

Purpose of the Study:

To determine the prevalence, characteristics, and natural history of LV involvement in RVC.
To better understand the progression of RVC and its impact on the LV.

Main Methods:

Studied 39 patients with RVC using 2-dimensional echocardiography.
Categorized patients into groups based on initial LV involvement (normal vs. abnormal).
Monitored patients during follow-up and analyzed pathological findings in available cases.

Initially, 25 patients had isolated RV abnormalities, while 14 showed concurrent LV involvement.

During follow-up, 6 new cases of LV involvement emerged, and 4 worsened.

Pathology revealed myocyte atrophy and fibro-fatty replacement in the RV, and fibrosis in the LV.

Conclusions:

Left ventricular involvement is frequent in right ventricular cardiomyopathy.
The disease progression indicates that RVC can significantly affect LV function over time.
This underscores the importance of comprehensive cardiac assessment in RVC patients.
