Summary

Monthly high-dose intravenous gamma globulin (IVGG) plus methylprednisolone did not prevent disease exacerbations in most progressive multiple sclerosis patients. The combination therapy was safe but failed to show a steroid-sparing effect.

Background:

  • Progressive multiple sclerosis (MS) is characterized by disease exacerbations.
  • Steroid dependence is common in MS patients.
  • Intravenous gamma globulin (IVGG) is explored as a potential treatment to reduce MS relapses.

Purpose of the Study:

  • To evaluate the efficacy of monthly high-dose IVGG combined with methylprednisolone in preventing disease exacerbations in progressive MS.
  • To assess the safety and tolerability of this combination therapy over a prolonged period.

Main Methods:

  • 14 patients with progressive MS received monthly IVGG (0.5-2 g/kg) and methylprednisolone.
  • 11 of these patients were steroid-dependent.
  • Follow-up was conducted over a mean period of 7.8 months to monitor disease activity.

  • 17 exacerbations occurred in 11 patients during the follow-up period.
  • Four exacerbations happened within one month of high-dose IVGG administration (1.6-2.0 g/kg).

  • Monthly IVGG and methylprednisolone can be safely administered for extended durations in progressive MS.
  • The dosage used did not prevent exacerbations in 80% of the studied patients.
  • Further research may be needed to optimize IVGG dosage or identify patient subgroups who might benefit.
