Background:

  • Merkel cell carcinoma (MCC) is a rare, aggressive skin cancer.
  • MCC commonly recurs locally and can metastasize intra-abdominally, primarily to the liver and retroperitoneal lymph nodes.

Observation:

  • A patient with known Merkel cell carcinoma of the right buttock presented with acute upper gastrointestinal bleeding.
  • Endoscopic examination revealed metastases to the stomach and duodenum.

Findings:

  • This case demonstrates unusual intra-abdominal spread of Merkel cell carcinoma to the upper gastrointestinal tract.
  • Metastasis to the stomach and duodenum from MCC can manifest as significant gastrointestinal hemorrhage.
