Summary

This study found no significant differences in common immunological markers between infertile and fertile couples. However, fertile couples more frequently showed specific antibodies, suggesting current tests may not fully capture immunological infertility causes.

  • To determine the frequency of immunological factors in couples with unexplained infertility.
  • To compare immunological marker prevalence between infertile and fertile control couples.
  • To assess the diagnostic utility of established immunological assays for infertility.

  • Studied 92 couples with unexplained infertility and 41 fertile control couples.

  • Assayed for anti-sperm antibodies (ASA), human leukocyte antigen (HLA) types, lymphocytotoxic antibodies (LCA), and inhibitors of mixed lymphocyte culture (MLC) and mouse blastocyst assay (MBA).
  • Also tested 73 women for antiphospholipid antibodies (APA).
  • Main Results:

    • No significant differences were found in the frequencies of APA, ASA, MBA, and HLA associations between infertile and fertile couples.
    • Fertile couples exhibited significantly higher rates of wife anti-husband LCA (51% vs. 8%) and MLC inhibitors (29% vs. 4%) compared to infertile couples.
    • Established markers like APA, ASA, HLA, LCA, MLC, and MBA showed limited discriminatory power in this cohort.

    Conclusions:

    • The study suggests that commonly tested immunological markers may not be reliable indicators of infertility in the studied population.
    • The higher prevalence of specific alloimmune markers (LCA, MLC inhibitors) in fertile couples warrants further investigation.
    • More specific immunological markers are required for accurate diagnosis of immunological infertility.
