Waldenström's macroglobulinemia (WMG) rarely involves the airways. This case highlights endobronchial tumor cell invasion causing airway narrowing and respiratory symptoms in WMG patients.

  • The tumor invasion led to significant narrowing of the patient's airways.

  • Endobronchial involvement is a previously undescribed manifestation of Waldenström's macroglobulinemia.
