Background:

  • Idiopathic acute eosinophilic pneumonia (IAEP) is a recently described, rare respiratory condition.
  • Characterized by acute onset of respiratory symptoms and pulmonary infiltrates.
  • Often presents with significant eosinophilic infiltration in the lungs.

Observation:

  • A previously healthy young male presented with acute respiratory distress and high fever.
  • Chest radiography revealed bilateral ground-glass opacities.
  • Bronchoalveolar lavage showed marked eosinophilia (72%) without infection.

Findings:

  • Transbronchial lung biopsy confirmed significant eosinophil infiltration in bronchiolar walls and alveolar septa.
  • The clinical presentation and biopsy findings were consistent with IAEP.
