Summary

Patients with end-stage renal failure (ESRF) show decreased vitamin C levels, particularly those on conservative therapy. The dehydroascorbic acid/ascorbic acid ratio may indicate overall vitamin C status in ESRF patients.

Area of Science:

  • Biochemistry
  • Nephrology
  • Nutritional Science

Background:

  • End-stage renal failure (ESRF) impacts nutrient metabolism.
  • Vitamin C (ascorbic acid) status is crucial for overall health.
  • Assessing vitamin C levels in ESRF patients requires understanding its different forms.

Purpose of the Study:

  • To compare plasma levels of ascorbic acid (AA) and dehydroascorbic acid (DHA) in ESRF patients and healthy controls.
  • To investigate the relationship between dietary vitamin C intake and plasma levels in ESRF.
  • To evaluate the dehydroascorbic acid/ascorbic acid ratio as an indicator of vitamin C status in ESRF.

Main Methods:

  • Plasma AA and DHA levels were measured in ESRF patients (conservative therapy and maintenance hemodialysis) and healthy controls.
  • Dietary vitamin C intake was assessed.
  • The DHA/AA ratio was calculated and analyzed.

Main Results:

  • Dietary vitamin C intake was significantly lower in ESRF patients on conservative therapy compared to controls.
  • Plasma AA levels were significantly reduced in ESRF patients on conservative therapy and moderately reduced in those on maintenance hemodialysis.
  • Plasma DHA levels showed a non-significant increase in ESRF groups, and the DHA/AA ratio was inversely proportional to plasma AA.

Conclusions:

  • ESRF patients, especially those on conservative management, exhibit impaired vitamin C status.
  • The DHA/AA ratio may serve as a valuable indicator of the body's overall vitamin C status in ESRF.
  • Further research is warranted to explore vitamin C supplementation strategies in ESRF management.
