Summary

Continuing antiplatelet therapy during cataract surgery is generally safe, with no serious complications observed. However, larger studies are needed to confirm the safety of antiplatelet medications.

Background:

  • Cataract surgery is a common procedure.
  • Antiplatelet therapy is often prescribed for cardiovascular health.

Purpose of the Study:

  • To evaluate the safety and complications of phacoemulsification with intraocular lens implantation in patients continuing antiplatelet therapy.

Main Methods:

  • Prospective, nonrandomized study.
  • Compared 24 patients on antiplatelet therapy with 36 patients not on antiplatelet therapy.
  • Monitored for hemorrhagic complications, visual acuity, and intraocular pressure.

Main Results:

  • Postoperative subconjunctival hemorrhages were more frequent in the antiplatelet group (P = .001).
  • One limited peribulbar hemorrhage occurred in the antiplatelet group; one hyphema in the non-antiplatelet group.
  • No serious complications or differences in visual acuity or intraocular pressure were noted between groups.

Conclusions:

  • Phacoemulsification and intraocular lens implantation can be safely performed in some patients continuing antiplatelet therapy.
  • The study sample size was insufficient to rule out an increased risk of rare but serious hemorrhagic complications.
  • Further research with larger cohorts is warranted to definitively assess the risks associated with antiplatelet therapy during cataract surgery.
