A rare, large extra-pleural hematoma associated with a thoracic vertebra fracture was identified. Computed tomography (CT) scans accurately diagnosed this condition, differentiating it from hemothorax.

Area of Science:

  • Trauma Surgery
  • Diagnostic Imaging
  • Thoracic Medicine

Background:

  • Extra-pleural hematomas are rare, potentially life-threatening collections of blood.
  • Associated with thoracic trauma, they can be misdiagnosed on initial imaging.

Observation:

  • A 74-year-old male presented with a voluminous extra-pleural hematoma following craniothoracic injury.
  • Conventional radiography initially suggested hemothorax, but thoracic CT revealed a distinct posterior parietal mass.

Findings:

  • Thoracic CT demonstrated a homogeneous mass with fatty lining adjacent to an eleventh thoracic vertebra fracture.
  • CT imaging proved crucial in revising the diagnosis from hemothorax to extra-pleural hematoma.

Implications:

Related Experiment Videos

  • Accurate diagnosis via CT is vital for managing significant extra-pleural hematomas.
  • Urgent surgical intervention is indicated for large hematomas due to their high morbidity.
  • The pleural lining sign on CT is a reliable indicator for diagnosing this rare condition.
