Summary

Acute lymphoblastic leukemia can affect vision by infiltrating optic nerves. Prompt radiotherapy to the brain and orbit successfully treated both initial and recurrent leukemic optic nerve involvement.

Background:

  • Acute lymphoblastic leukemia (ALL) is a hematologic malignancy that can infiltrate various organs.
  • Leukemic infiltration of the central nervous system, including the optic nerves, can lead to visual disturbances.

Observation:

  • A 40-year-old woman with ALL presented with acute visual disturbance in the right eye (OD).
  • Funduscopic examination revealed mild optic disc swelling, and cerebrospinal fluid analysis confirmed the presence of blast cells.

Findings:

  • Initial radiotherapy to the brain and orbit resolved the ocular and neurological findings.
  • Five months later, the patient experienced decreased visual acuity in the left eye (OS), indicating recurrent leukemic optic nerve involvement.
  • Subsequent radiotherapy effectively treated the recurrent leukemic infiltration of the optic nerve.
