Summary

Background:

Metastatic renal cell carcinoma (RCC) presents a significant treatment challenge.
Interferon-alpha (IFN-alpha) family includes novel subspecies like recombinant interferon (rIFN) alpha-C with high specific activity.
Previous treatments for advanced RCC have shown limited efficacy.

Purpose of the Study:

To summarize 3-year clinical experience with rIFN-alpha C in metastatic RCC.
To assess the response rate and safety of low-dose rIFN-alpha C in a Phase II study.
To evaluate the efficacy of vinblastine as an add-on therapy to IFN for refractory RCC.

Main Methods:

A Phase II clinical study involving 33 patients with metastatic RCC treated with rIFN-alpha C.
Dosage regimen: 3 x 10(6) U daily, then 3 x 10(6) U/m2 every other day.
A subgroup of nine patients received vinblastine after failing IFN monotherapy.

Main Results:

Partial remission rate of 9.7% and stable disease rate of 25.8% observed in 33 patients.
rIFN-alpha C was generally well-tolerated with mild side effects.
Five patients experienced neurotoxicity, including mental deterioration and behavioral changes.
Vinblastine added to IFN therapy showed no response in nine patients with refractory RCC.

Conclusions:

rIFN-alpha C demonstrates moderate clinical activity in metastatic RCC.
Awareness of potential neurotoxicity is crucial for effective patient management.
Vinblastine appears to have limited benefit as second-line therapy in combination with IFN for RCC.
